Forms

You may fill in an on-line form, or download a form (in Microsoft Word format), and attach it to an email. Email is a good option when you want to include attachments. The downloadable forms also include information on who should receive them.

To download a form, right-click on it and select the 'save target as...' option. Choose the folder where you'd like to save it and click on 'Save'. Then, open your file, update it with your information and save it. Open your email tool, insert the correct address(es) attach your file and send it!
